I understand and acknowledge that my participation in the Titan Football Summer Camp organized by St. Agnes Catholic Parish involves inherent risks, including but not limited to physical injury. In consideration for being permitted to participate in the camp activities, I hereby agree to the following:

1. **Assumption of Risks:** I am aware that participating in football activities carries certain inherent risks of injury. I voluntarily assume all risks associated with my child’s participation, including any physical injury, harm, or loss that may occur during or as a result of my child’s participation in the camp.

2. **Waiver of Liability:** I hereby waive, release, and discharge St. Agnes Catholic Parish, its directors, officers, employees, coaches, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child during the camp activities or as a result of my child’s participation, whether caused by the negligence of the camp organizers or otherwise.

3. **Medical Treatment Authorization:** In the event of injury or illness, I authorize the camp staff to obtain medical treatment for my child and agree to be financially responsible for any costs associated with such treatment.

4. **Photography Release:** I grant permission to St. Agnes CYO to take photographs and/or videos of me during the camp and to use these materials for promotional purposes, including but not limited to newsletters, websites, social media, and other promotional materials.

5. **Compliance with Rules:** I agree that my child will abide by all rules and instructions provided by the camp organizers and understand that failure to do so may result in my child’s removal from the camp activities.
